What would you like the health professional assessing you to understand? Bring notes using words that feel natural to you; a concern does not need medical phrasing.
Make room for your history
Write down what brought you to the visit, the medicines you take, and past reactions you want to discuss. Add the questions that keep coming back. Ask what information the clinician needs before deciding.
An assessment needs more than a request
As of October 3, 2026, an Arizona medical doctor generally needs to examine you or have an established doctor-patient relationship before prescribing.
The law allows some exceptions, and a suitable examination may take place through telehealth.
The relationship in question is with the doctor writing this prescription. Tell that doctor whether they have cared for you before. Ask: Will you examine me? If not, what care relationship or exception allows you to prescribe in my case? Please explain how it applies to me.
Medical-board guidance says a care relationship can start through telehealth when the care meets professional standards.
Having a different doctor elsewhere does not answer how this doctor will assess you.

Questions can go both ways
The Federation of State Medical Boards rejects prescriptions based solely on fixed forms or patient-picked treatment menus, in guidance checked September 26, 2026.
Patients must also have a route for follow-up questions, according to that guidance.
That group brings state medical boards together; its guidance is not itself an Arizona statute.

How will a needed test or examination be arranged?
The medical-board group's guidance says to arrange needed tests or refer the patient when a remote assessment needs more information.
Ask who will arrange the next step, who will review the result, and what it costs. Ask what follow-up would look like if no medicine is prescribed.
Leave with a clear next step
Can you explain what you found, what choices we have, and when we will speak again? That question also leaves room to discuss an option without a medicine.
